Clinical use of organic near‐infrared fluorescent contrast agents in image‐guided oncologic procedures and its potential in veterinary oncology

Abstract: One of the major challenges in surgical oncology is the intraoperative discrimination of tumoural versus healthy tissue. Until today, surgeons rely on visual inspection and palpation to define the tumoural margins during surgery and, unfortunately, for various cancer types, the local recurrence rate thus remains unacceptably high. Near-infrared (NIR) fluorescence imaging is an optical imaging technique that can provide real-time preoperative and intraoperative information after administration of a fluorescent … Show more

“…Better diagnostic accuracy and more extensive fluorescent‐guided cytoreductive surgery were recently described in human patients with peritoneal carcinomatosis after fluorescent contrast administration 24‐26 . A combination of different diagnostic and therapeutic options might prove valuable in the future by using already existing techniques such as fluorescence imaging for both sentinel lymph node mapping 27 and carcinomatosis detection. Although the carcinomatosis was observed only after the RARP, it is impossible to trace whether this unwanted outcome was triggered by the surgery or not.…”

“…Various methods of preoperative and intraoperative SLN mapping have been described including lymphoscintigraphy, 9,22,23 injection of vital dyes such as methylene blue, isosulfan blue, or patent blue violet, 5,9,15,[23][24][25][26] contrast enhanced ultrasonography, 27,28 nearinfrared florescent imaging, 5,28,29 lipid soluble iodinated contrast medium with radiography or CT, 15,18,30,31 and water soluble iodinated contrast medium (WIC) with CT. 16,17,19,20,31 The most common and current gold standard modality for SLN mapping in both human and veterinary oncology is lymphoscintigraphy that uses a radiolabeled colloid injected subcutaneously around the primary tumor site followed by assessment of the movement of radioactive particles through the lymphatic drainage system using a gamma camera. Unfortunately, lymphoscintigraphy is costly and requires special licensure for the use of a radioactive isotope and specialized equipment that limits availability of this diagnostic technique to referral centers.…”

“…Near-infrared fluorescence (NIRF) imaging is a relatively novel technology that has been shown effective during tumour resection. [18][19][20][21][22][23] Visible light only penetrates a few hundred micrometers into blood and tissue and provides only information of the surface features of the surgical field. The nearinfrared window (700-950 nm) lies outside the human visible wavelength range 24 and allows deep tissue imaging (up to 15 mm 16 19 25 26 ).…”

Intraoperative tumour detection and margin assessment is one of the most promising applications of the NIRF technology,23 since it may increase the likelihood of complete eradication of tumour tissue and thus reduce the risk of tumour recurrence.…”
