“…In a recent study by Zheng et al, focused on the PERSONS cohort, 9 of the 780 consecutively enrolled subjects with liver biopsy, 663 (85%) fulfilled both the "NAFLD" and "MAFLD" criteria. 10 The difference in the number between the two groups (670 NAFLD vs. 773 MAFLD) was due to 7 patients with FLD who were lean by body mass index (BMI) without satisfying the metabolic risk threshold (defined as Non-metabolic risk NAFLD, Non-MR NAFLD), and the inclusion of 110 patients with FLD and other etiologies of liver diseases (66 with excess alcohol consumption, 40 with viral hepatitis, and 4 with autoimmune hepatitis). Notably, Age, sex, comorbid diseases (i.e., diabetes mellitus and hypertension), variant distribution of the PNPLA3 gene, metabolic profile, and histology including liver steatosis and fibrosis were not significantly different between the two groups.…”

“…However, some studies do not report this finding; one study (7) of 1,710 persons from the general population found that advanced liver fibrosis (defined as a median liver stiffness ≥9.7 KPa) was not different in MAFLD and NAFLD groups. Another study (8) of a population of 780 with liver biopsy reported that both groups have similar clinical characteristics and liver histology. Likewise, a cohort study (4) with a 7-year follow-up of ~900 patients showed that MAFLD and NAFLD groups had similar new-onset cardiovascular events.…”
