Clinical Utility of TSH Receptor Antibodies

Abstract: Third-generation TRAb assays are suitable in the differential diagnosis of hyperthyroidism. In GD, TRAb should be tested before deciding whether methimazole can be stopped. TRAb should be used in pregnant women with GD to assess the risk of fetal thyrotoxicosis. The use of TRAb in GO requires further studies.

“…There is no correlation between maternal thyroid function and maternal serum TRAb concentration in such cases, and the impact of maternal TRAbs on the foetus is uncertain. Our opinion is consistent with other authors [60], who postulate performing TRAb and complementary thyroid stimulating immunoglobulin tests. Similar to previous studies [35], maternal ATD therapy, and low maternal serum TRAb and fT4 levels turned out to be the risk factors for foetal hypothyroidism in our material.…”

“…The following exclusion criteria were applied: type 2 or other types of diabetes; currently in the honeymoon period, which was defined as a daily insulin requirement <0.5 IU/kg/day and an HbA 1c level <7% (53 mmol/mol);a diabetes duration of <1.5 years; abnormal thyroid function; elevated creatinine level; using contraceptive pills, steroids or any other type of medication, and the presence of other chronic conditions. Autoantibodies were not assessed as inclusion criteria because they exhibit around 20-30% negativity in young Chilean patients with T1D [20,21]. …”
