Clinical value of gallium‐67 SPECT scintigraphy in the diagnostic and therapeutic evaluation of retroperitoneal fibrosis: a prospective study

Abstract: Abstract. van Bommel EFH, Siemes C, van der Veer SJ, Han SH, Huiskes AWLC, Hendriksz TR (Albert Schweitzer Hospital, Dordrecht, The Netherlands). Clinical value of gallium-67 SPECT scintigraphy in the diagnostic and therapeutic evaluation of retroperitoneal fibrosis: a prospective study. J Intern Med 2007; 262: 224-234.Background. Gallium-67 ( 67 Ga) scintigraphy may be useful in evaluating patients with retroperitoneal fibrosis (RPF), but a systematic assessment of its value is lacking.

“…At present, there are no validated predictors of response to glucocorticoids. Gallium scanning has been proposed to help in predicting such a response (21), but a recent study showed that such scanning failed to discriminate between responders and nonresponders to prednisone (15). There is a general belief that an elevated ESR and an elevated CRP level at the time of diagnosis reflect active inflammation, indicating that glucocorticoid therapy might be effective.…”

“…Scintigraphy using gallium-67 is positive in about 70% of patients with retroperitoneal fibrosis and is used to assess recurrent disease in these patients. 8 Positron emission tomography using fluorine-18 fludeoxyglucose is also useful for monitoring active inflammation within the fibrotic soft tissue. 9…”

“…Repeat CT scanning showed a large peri-aortic mass; gallium-67 scanning revealed intense pathologic activity. 46 Treatment was started with prednisone and cyclophosphamide, which led to rapid improvement. At the time of his latest follow-up, a second recurrence after surgical intervention necessitated re-introduction of immunosuppressive treatment.…”

“…Gallium scanning was negative. 46 Recurrent chronic peri-aortitis despite aneurysmectomy was suspected. Chronic kidney disease was thought to be due to chronic hypertension and generalized vascular disease.…”
