Clinically node negative breast cancer patients undergoing breast conserving therapy, sentinel lymph node procedure versus follow-up: a Dutch randomized controlled multicentre trial (BOOG 2013-08)

Abstract: BackgroundStudies showed that axillary lymph node dissection can be safely omitted in presence of positive sentinel lymph node(s) in breast cancer patients treated with breast conserving therapy. Since the outcome of the sentinel lymph node biopsy has no clinical consequence, the value of the procedure itself is being questioned. The aim of the BOOG 2013–08 trial is to investigate whether the sentinel lymph node biopsy can be safely omitted in clinically node negative breast cancer patients treated with breast… Show more

“…In contrast to SOUND and INSEMA, patients with neoadjuvant chemotherapy are included as well. The primary endpoint is the regional recurrence rate after 5 years; the estimated sample size is 1,644 patients [51]. The current status of recruitment is n = 506 (March 2018).…”

“…The SOUND trial randomizes women with cN0 breast cancer who are treated with breast‐conserving surgery and radiotherapy to SLNB with, or without ALND, or no surgical staging of the axilla. In the BOOG 2013‐08 trial, women with cT1–2 N0 disease who have breast‐conserving surgery are randomized to SLNB or no SLNB. Women treated with NACT are also eligible for inclusion in this trial, regardless of the timing of SLNB.…”

“…Up to now, only 2 trials evaluated accuracy of such a combination procedure and were included in our meta-analysis. 6,32 This procedure is associated with excellent IFRs. Caudle et al 6 confirmed that the clipped node does not necessarily have to be a SLN, as this was the case in only 77%.…”

“…A total of 17 trials investigated accuracy of SLNB, 15-31 1 trial investigated ML, 8 and 2 trials investigated a combination procedure. 6,32 Studies validating the combination procedure were scarce, yet several studies did report on cohorts of patients in whom a combination procedure was performed without routine cALND. Therefore, we decided to include these studies, 7 in total, in the qualitative analysis.…”
