“…In the present study as well, multiple intrahepatic recurrences considered intrahepatic metastases after RFA occurred more frequently in the hypointense group than in the non‐hypointense group. On the other hand, Yamamoto et al . reported that the frequencies of poor differentiation and portal vein invasion were significantly higher for locally recurrent HCC after RFA than after surgical resection.…”

“…The phenotype of early-recurring tumors and the surrounding non-cancerous region has been described in several reports. An immunohistochemical analysis of tumors that recurred within 1 year after radical RFA showed that these tumors had a higher frequency (67%) of poorly differentiated type and higher rates of positivity for the proliferation marker Ki-67 than non-recurrence within 1 year, with a lower rate of positivity of the negative cell cycle regulator p27 Kip1 [143]. A genome-wide gene-expression profile of resected cancer tissue and the surrounding noncancerous liver tissue showed that the increased expression of the cytochrome P450 1A2 (CYP1A2) gene in noncancerous tissue was a predictive marker for non-recurrence [144].…”

“…3) in caso di recidiva locale dopo termoablazione è raccomandabile considerare un trattamento più aggressivo, ossia la resezione o il trapianto. Infatti, non solo vi è stato il fallimento di un trattamento pregresso (a patto che sia stato eseguito in condizioni ottimali), ma la letteratura descrive una maggiore aggressività biologica della recidiva post-termoablazione (323)(324)(325)(326).…”
