Clomiphene citrate-induced visual hallucinations: a case report

Abstract: Background: Polycystic ovary syndrome is a common cause of chronic anovulation and infertility in otherwise healthy fertile couples. Clomiphene citrate is used as a first-line ovulation induction therapy in patients with polycystic ovary syndrome. Clomiphene citrate can cause both systemic and ocular side effects. We report a rare side effect of illusory palinopsias in a patient with polycystic ovary syndrome treated with ovulation induction therapy with clomiphene citrate, and emphasize the need for gynecolog… Show more

“…The pathophysiology of palinopsia is not clear, but many hypothesis are proposed. One of them is that drugs could alter serotoninergic activity ( 12 14 ) or inhibit the synthesis of gamma-aminobutyric acid involved in the generation of action potentials of the visual system ( 16 , 17 ). Another involves a local irritation of the occipito-parietal cortex by surrounding oedema ( 18 ).…”

“…However, in our case the symptoms lasted for < 10 minutes and therefore it was practically not possible for her to undergo fMRI. Visual hallucinations following treatment with ovulation induction drugs like clomiphene citrate due to a similar pathomechanism has also been reported [9, 10]. Keratoconus progression due to increase in estrogen levels following IVF treatment has been reported by Yuksel et al [5].…”

“…According to our method, these ADRs can all be predicted at the threshold = 0.8, and faint and visual disturbances are new side effects clomifene may produce. It has been reported there are several cases that visual complaints may occur during treatment course with clomiphene citrate, such as sudden visual field shimmering, visual hallucinations, and so on. , Diflorasone (DB00223) is a topical corticosteroid used to treat itching and inflammation of the skin (Figure B). Like other topical corticosteroids, diflorasone has anti-inflammatory, antipruritic, and vasoconstrictive properties.…”
