Clonal Spread of Levofloxacin-Resistant Streptococcus pneumoniae Invasive Isolates in Madrid, Spain, 2007 to 2009

Abstract: Among 1,349Streptococcus pneumoniaeinvasive isolates, 45 (3.3%) were levofloxacin resistant. Serotype distribution was as follows: 8 (n= 32 isolates), 19A (n= 4 isolates), 7F (n= 3 isolates), 9V (n= 2 isolates), 10A (n= 1 isolate), 19F (n= 1 isolate), 6B (n= 1 isolate), and nontypeable (n= 1 isolate). Levofloxacin-resistant isolates had dual mutations in thegyrAandparCgenes. Serotype 8 strains corresponded to a capsular switching of the Sweden15A-25 clone. Levofloxacin resistance was also detected among multir… Show more

“…All serotype 8 isolates were associated with genotype CC63, suggesting a capsular switching event. This clone expressing serotype 8 was first detected in the 2006 study (18), and it was disseminated in the Madrid area, mainly among HIV-positive patients, with one isolate showing FQ resistance and a ParC S79F amino acid substitution (26,41). In the present series, eight CC63-serotype 8 isolates were also isolated from patients at hospitals in Madrid, a fact that could explain the frequency of this serotype among Cip r pneumococci in 2012.…”

“…These cases tend to involve elderly patients with chronic respiratory diseases, such as chronic obstructive pulmonary disease (COPD), in which higher rates of FQ resistance have been detected (24). Although the development of FQ resistance has been associated with FQ consumption (14,22,25), the dissemination of pneumococcal FQ-resistant clones has rarely been observed so far (26). However, previous epidemiological studies (17,18) have re-vealed a low genetic diversity of pneumococcal clones among FQresistant pneumococci in Spain.…”

“…Other species, such as Haemophilus spp. and ␤-hemolytic streptococci, have been less prone to becoming fluoroquinolone nonsusceptible but are emerging at high rates in some countries (25)(26)(27)(28)(29)(30)(31). Hampering the advancement of fluoroquinolones to include other more potent agents has been difficult, due to the safety profile observed for this class.…”
