Coadministration of Anastrozole Sustains Therapeutic Testosterone Levels in Hypogonadal Men Undergoing Testosterone Pellet Insertion

Abstract: Introduction Current U.S. Food and Drug Administration–approved therapies for hypogonadism involve testosterone (T) replacement. Testosterone pellets (TP) require a minor office procedure every 3 to 4 months. The need for repeated insertions increases the likelihood of a complication. Anastrozole (AZ) is an aromatase inhibitor that has been used off-label for the treatment of male hypogonadism. AZ increases T levels by lowering serum estradiol (E2) levels and increasing gonadotropin (GTP) lev… Show more

“…The latter may have special clinical relevance [ 54 ], especially since aromatase inhibitors, used to suppress estrogen levels in patients with ER+ breast tumors, can raise testosterone levels. As an example, anastrozole increases testosterone levels by decreasing serum estradiol levels [ 55 ]. Finally, PIK3CA/AKT/mTOR signaling is ubiquitously deregulated in cancer, and it is apparent that there is significant crosstalk between this pathway and that related to androgens (Figure 3 ).…”

“… 5 While AZ is commonly used as an adjunctive therapy to increase T levels in men whom spermatogenic preservation is important and therefore avoidance of exogenous testosterone is crucial, in our series AIs did not impact testosterone levels. However, other studies suggest coadministration of an AI with exogenous TTH increases serum T. 10 All of these received T pellets, suggesting a differential response based on TTH type.…”

“…Men on Testopel® and anastrozole did not demonstrate the secondary increase in their estradiol levels or suppression of their LH and FSH levels. The addition of a generic aromatase inhibitor significantly decreased the number of annual Testopel® insertions [ 18 •].…”
