Cocaine-Induced Coronary-Artery Vasospasm

Abstract: T h e ne w e ngl a nd jou r na l o f m e dicine n engl j med 374;5 nejm.org February 4, 2016 e5 Images in Clinical MedicineA 58-year-old woman presented to the emergency department with intermittent, nonexertional chest pain of 2 days' duration, with each episode lasting 15 minutes. The pain was relieved by sublingual nitroglycerin and acetaminophen with hydrocodone. Her medical history was notable for tobacco and alcohol use, hypertension, and hyperlipidemia. She had been admitted for similar symptoms 2 month… Show more

“…There have been reports on the use of benzodiazepines, alpha-adrenoceptor blocking drugs, alpha-2-adrenoceptor agonists, beta blockers, antipsychotics, and morphine, but high-quality evidence for the treatment of cocaine-induced cardiovascular toxicity is limited 14 . In some reports, coronary vasospasm could be reverted by intracoronary injection of nitroglycerine 15 , 16 . In our patient, massive coronary vasospasm occurred 1 h after nitroglycerine administration, which underlines the importance of continuous administration of vasodilators in this setting.…”

“…Evidence suggests that this risk is lower than hypothesized, and this is still a debate within the field. [264][265][266] Shin et al (2019) conducted a systematic review and meta-analysis on the use of beta blockers to treat cocaine intoxication and cocaineassociated chest pain, finding that beta blockers were not associated with adverse events-including myocardial infarction (MI), myocardial necrosis, or death-during hospitalization and long-term follow-up. 267 However, this issue remains an area of controversy in the field.…”

“…Unopposed alpha stimulation can result in an acute increase in blood pressure and/or coronary artery vasoconstriction following beta blocker administration. Evidence suggests that this risk is lower than hypothesized, and this is still a debate within the field 264-266 . Shin et al (2019) conducted a systematic review and meta-analysis on the use of beta blockers to treat cocaine intoxication and cocaine-associated chest pain, finding that beta blockers were not associated with adverse events—including myocardial infarction (MI), myocardial necrosis, or death—during hospitalization and long-term follow-up 267 .…”
