Cochrane review: Antihistamines and/or decongestants for otitis media with effusion (OME) in children

Abstract: BackgroundOtitis media with effusion (OME) is common and may cause hearing loss with associated developmental delay. Treatment remains controversial. The effectiveness of antihistamines, decongestants and antihistamine/decongestant combinations in promoting the resolution of effusions has been assessed by randomized controlled trials.ObjectivesThe objective of this review is to determine whether antihistamine, decongestant, or combination therapy is effective in treating children who present with OME.Search st… Show more

“…13 Unfortunately, all available medical treatments for otitis media with effusion such as antibiotics, antihistamines, decongestants and intranasal steroids are ineffective and have unwanted effects, and therefore cannot be recommended. [21][22][23] Not only are antibiotics ineffective, but resistance to them poses a major threat to public health. 24,25 Although surgery is effective for a carefully selected minority, 13,26,27 a simple low-cost, nonsurgical treatment option could ben-…”

“…Moreover, there was a 5-8 fold increased risk of side effects for those receiving an intervention. Griffin and Flynn, 2011 emphasized that there is no benefit and some harm from the use of antihistamines or decongestants alone or in combination in the management of otitis media with effusion 18 . We also discovered that there is no significant difference from the use of topical nasal decongestant in the treatment of AOM.…”

“…45 Tracy and Demain 46 compared the effects of prophylactic antibiotics alone or in combination with intranasal beclomethasone or placebo and demonstrated that subjects in the beclomethasone plus antibiotic group improved more rapidly than those in the antibiotic alone or placebo group.Nevertheless, no differences were seen amongsubjects with and without allergy. Chimona et al 47 assessed the role of antihistamines in OME in an animal experimental model, but no clinical efficacy has been demonstrated for antihistamine alone or in association with topical decongestantsin humans 48 , as confirmed by a meta-analysis. 49…”
