Coeliac disease and small bowel carcinoma

Abstract: Summary Small bowel carcinoma can occur in association with villous atrophy in the jejunum. In most reported cases there is doubt as to which of these represents the primary lesion. These cases are reviewed here. A further case is reported in which a carcinoma developed in a patient known to be suffering from coeliac disease for some years, although he was not on a gluten-free diet for the time preceding the diagnosis of carcinoma. On dietary restriction he remains well at 2-year follow-up. Poss… Show more

“…The association between CD and SBA was first described more than 40 years ago [9] and, in contrast to the general population, CD patients have a markedly increased risk of developing SBA. Nowadays it is largely agreed that a delay in CD diagnosis and a low compliance with gluten free diet (GFD) are two major factors leading to a 60–80 fold relative risk of developing SBA [1012].…”

“…4 Similar to the situation in the large bowel, there is evidence that small bowel adenocarcinomas also arise from pre-existing adenomas. 5 6 Although there have been many reported cases of small bowel adenocarcinoma developing in coeliac disease patients, [7][8][9][10][11][12][13][14][15] there has been only one published case of a small bowel adenoma in a patient with coeliac disease. 16 Within our cohort of patients with coeliac disease, we have encountered three patients with a small bowel adenoma and four with adenocarcinoma of the small bowel.…”

“…Accordingly, among all SBC, 13% are associated with a diagnosis of CD [48]. The first case of SBC in CD was described in 1972 and, from that on, many other reports followed up [61].…”
