Coexistence of chronic myelogenous leukemia and chronic lymphocytic leukemia

Abstract: A 55‐year‐old man is reported who initially developed chronic lymphocytic leukemia. Seven years later, after chemotherapy with chlorambucil, chronic myelogenous leukemia was diagnosed in addition to the chronic lymphocytic leukemia. Four previously reported cases with the same sequence of events are reviewed as well as cases of chronic myelogenous leukemia following chemotherapy alone.

“…The spectrum of secondary solid neoplasms during the course of CLL includes carcinomas of the skin, stomach, colon, breast, or kidney [4, 5]; among secondary hematologic malignancies, cases of acute lymphocytic, myelocytic, monocytic and myelomonocytic leukemia, erythroleukemia, plasma cell leukaemia [6], and myeloproliferative neoplasms (MPN) have been reported [7–54].…”

“…A higher frequency of melanomatous and nonmelanomatous skin cancer, as well as of lung cancer and adenocarcinoma of the GI tract, has also been described [7]. Although the majority of patients with CLL who developed CML had received either RT or alkylating agents (chlorambucil) (np8) [4,26,62,78,102,109,123,124], four cases of secondary CML in untreated patients have also been reported (Table 2) [45,92,123,124]. The median interval to development of CML was 30 months (range 17-53 months), which is similar to that reported in a recent study on HCL patients treated for their disease, with a peak incidence of secondary malignancies within the first 2 years after ID of HCL [7].…”

“…However, only two were treated with CT alone. Apart from these two cases another 12 patients with tr-CML after CT alone have been reported (Table 4) [4,26,43,62,69,85,95,102,109,117,121,124]. The median time from ID of the primary malignancy to CML was 66 months (range 18-150 months).…”
