Cognitive-Behavioral Analysis System of Psychotherapy as a Maintenance Treatment for Chronic Depression.

Abstract: Although the efficacy of maintenance pharmacotherapy for the prevention of recurrence in major depressive disorder (MDD) is well documented, few studies have tested the efficacy of psychotherapy as a maintenance treatment. The authors examined the efficacy of the cognitive-behavioral analysis system of psychotherapy (CBASP) as a maintenance treatment for chronic forms of MDD. Eighty-two patients who had responded to acute and continuation phase CBASP were randomized to monthly CBASP or assessment only for 1 ye… Show more

“…Averaging separately by treatment type yielded relapse-recurrence rates of 12% (1.2% per month; C-CT) and 38% (4.0% per month; nonactive controls) over a mean of 41 weeks. One included study, Klein et al (2004), tested a form of maintenance CT by excluding patients who relapsed during a continuation phase of the same treatment. The other included studies (Jarrett et al, 1998(Jarrett et al, ,2000(Jarrett et al, ,2001) evaluated C-CT implemented immediately after A-CT.…”

“…In the 16-week continuation phase of treatment, 57 a greater number of patients maintained partial or full remission on the combination treatment (90%) as compared with the pharmacotherapy (80%) or psychotherapy alone (82%). In the 1-year maintenance phase of treatment, 58 patients in the CBASP group had significantly fewer recurrences (p ≤ .03) than those patients in the assessment-only group. Patients who receive psychotherapy in the acute and continuation phases of treatment and achieve remission may be at a lower risk for recurrence during the maintenance phase of treatment than those patients who do not receive psychotherapy.…”

“…Zajecka: In the early phases of treatment, CBT may prevent relapse, even in patients who discontinue their medication. 54,55 Dr. Keller: Compelling evidence for the value of psychotherapy is provided by a 3-phase study-acute, continuation, and maintenance treatment phases [56][57][58] -that used a modified form of CBT called CBASP. In the 12-week acute phase of treatment, 56 patients randomly assigned to CBASP plus pharmacotherapy had a greater improve-ment in psychosocial functioning and higher remission rates than patients who received pharmacotherapy or psychotherapy alone.…”
