Cognitive Decline and Oral Health in Middle-aged Adults in the ARIC Study

Abstract: Even before dementia becomes apparent, cognitive decline may contribute to deterioration in oral health. This cohort study of middle-aged adults evaluated associations of six-year change in cognitive function with oral health behaviors and conditions in the Atherosclerosis Risk in Communities (ARIC) study. Cognitive function was measured at study visits in 1990-1992 and 1996-1998 with three tests: (a) Delayed Word Recall (DWR), (b) Digit Symbol Substitution (DSS), and (c) Word Fluency (WF). Cognitive decline … Show more

“…The present study provides the first evidence of a significant association of multiple tooth loss with VCI in subjects with acute stroke after adjusting for age, socio‐economic status and systemic risk factors. This finding is consistent with the results from several large cohort studies in Japan and the USA . Notably, the present study recruits 161 patients with acute ischemic stroke, which is different from previous ones involving middle‐aged or elderly community populations .…”

“…This finding is consistent with the results from several large cohort studies in Japan and the USA . Notably, the present study recruits 161 patients with acute ischemic stroke, which is different from previous ones involving middle‐aged or elderly community populations . Another study shows that tooth loss is significantly associated with cognitive impairment in the general population, while after adjusting for socio‐economic status this association no longer exists .…”

“…Similar combined scores have been calculated and used in other studies for analysis of cognitive function. 30,31 The change in the test score for the digit symbol was positively associated with increased total flavonol intake (P-values < .001). Change in word fluency was only marginally associated with flavonol intake (P = .027 for the unadjusted model and not significantly correlated for the adjusted models).…”

“…Our study may have been underpowered to detect an association between BGI and cognitive function because the mean differences between the two extreme BGI groups were relatively small, and cognitive change during the eight-year follow-up was modest (Supplemental material). However, it is also possible that subtle changes in cognitive function occurring prior to study baseline resulted in poor oral hygiene care, thereby increasing plaque level and the extent of bleeding on probing at baseline (35). …”
