Cognitive status among patients with chronic obstructive pulmonary disease

Abstract: PurposeWe investigated the association between cognitive impairment and chronic obstructive pulmonary disease (COPD), taking into account demographic and clinical variables evaluated during routine practice.Patients and methodsWe performed a post hoc analysis of a cross-sectional study that included subjects with stable COPD. Sociodemographic and clinical information was recorded using the Body mass index, airflow Obstruction, Dyspnea and Exacerbations index and the Charlson comorbidity index. Cognitive perfor… Show more

“…In the univariate analysis, patients with cognitive impairment had significantly worse CAT scores (25.2 [SD =6.9] versus 19.3 [SD =8.7], p <0.001). 31 It is important to point out that patients with severe cognitive impairment that prevents them from understanding and completing the questionnaires were not included in the study.…”

“…Roncero et al showed CI in 39.4% of stable COPD patients. This study was large (N = 940), but it only assessed cognition using the Mini-Mental State Examination (MMSE) [21], which is useful as a screening tool but not as a diagnostic tool [22]. Samareh Fekri et al likewise administered the MMSE to 87 COPD patients and 60 age-and gender-matched individuals.…”

“…Several studies showed an inverse relationship between CI and HRQoL as assessed with the COPD Assessment Test [21,60], the EuroQoL-5 dimensions questionnaire [21] or St George's Respiratory Questionnaire [22]. Then again, other studies did not confirm this relationship [19,62].…”

“…Cognitive impairment is multifactorial, but a history of cigarete smoking, aging and educational level are recognized as major determinants [25,26]. The origin of the cerebral dysfunction COPD -An Update in Pathogenesis and Clinical Managementin patients with COPD is still unknown, assuming the interference of several pathological relays: hypoxemia, oxidative stress, systemic inlammation, smoking, comorbidities, vascularmediated brain pathology, neurotransmiter metabolism in the central nervous system (CNS), a decrease in physical functioning, genetic and epigenetic factors.…”
