Colchicine Treatment of Urticarial Vasculitis

Abstract: A patient with clinically and histologically typical urticarial vasculitis has been treated with 0.5 mg of colchicine three times per day. This treatment resulted in both disappearance of urticarial skin lesions and amelioration of severe systemic symptoms such as abdominal pain, fever and arthralgia. Colchicine is proposed as an effective therapeutic agent for urticarial vasculitis if other drugs are ineffective, poorly tolerated or contraindicated.

“…Colchicine and dapsone have also been reported to be useful for refractory urticaria and urticarial vasculitis. 60,61 There have been some reports of the effectiveness of the antileukotriene agents in urticaria, 62,63 but extensive clinical trials have not been performed, so their usefulness is still questionable. Early case reports and a clinical trial of omalizumab in CU appear promising, but considerable additional study is needed.…”

“…Hydroxycholoroquine has been administered to a few patients with improvement in urticaria, arthralgia, and the restoration of CH50 levels toward the normal range (17,18,95). Methatrexate (96), colchicine (97,98), and dapsone (17,91,99,100), have been used in isolated patients. The combination of dapsone and pentoxifylline (101) has been used.…”

“…There are anecdotal reports of its efficacy in leukoctyoclastic vasculitis (Asherson et al, 1991;Callen & af Ekenstrom, 1987;Plotnick et al, 1989;Werni et al, 1986) and equally, of its lack of efficacy (Boom et al, 1988;Spann et al, 1986). Colchicine is extensively used in Behcet's syndrome.…”
