ABSTRACT. Objective. To characterize the features of cold urticaria in children, with particular focus on systemic reactions, because little pediatric data are available.Methodology. Chart reviews of 30 children <18 years old who were evaluated in the past 3 years at the Children's Hospital Allergy Program (Boston, MA) and a private allergy practice. Demographic, diagnostic, and therapeutic data were collected. Telephone interviews of patients and/or their parents were performed to obtain follow-up data.Results. Our data showed that the mean and median ages of onset were ϳ7 years. No secondary causes were found. One third of patients had anaphylactic reactions. These reactions could not be predicted based on available variables. Patients with negative cold-stimulation test (ice-cube challenge) at 10 minutes had similar symptoms and response to antihistamines as those patients with positive ice-cube-challenge test. In addition, our group of patients with cold urticaria had a strikingly high rate of asthma (46.7%) and allergic rhinitis (50%). The rate of family history of atopic diseases was even higher (89.3%).Conclusions. Cold urticaria occurs in children and may be associated with anaphylaxis. In our series, no secondary causes were found. All patients with cold urticaria and their parents should be cautioned regarding the risk of anaphylaxis and provided with an epinephrine autoinjector. Of patients with cold urticaria, Ͼ90% have idiopathic (essential) cold urticaria. [2][3][4] The remainder are mostly secondary to cryoglobulinemia. A rare form of the disease, known as delayed cold-induced urticaria, is characterized by the delayed expression of urticaria and angioedema 9 to 18 hours after cold exposure. It is probably inherited as an autosomal dominant condition. 5 Another syndrome known as familial cold urticaria is characterized by the development of intermittent rash (not urticaria), fever, arthralgia, and conjunctivitis 2.5 hours after generalized exposure to cold. It is inherited as an autosomal dominant condition. Patients with this syndrome were identified recently to have mutations in chromosome 1q44. 6 The prevalence and course of cold urticaria are not well defined. The most common method to confirm the diagnosis is the ice-cube-challenge test. It entails the application of an ice cube on the skin for nonstandardized time intervals followed by a period of rewarming. ÖZGiriş: Soğuk ürtikeri nadir görülen bir fiziksel ürtiker alt tipidir. Çalışmada soğuk ürtikerli hastaların özelliklerini, buz testinde provokasyon süresini uzatmanın testin tanısal değeri üzerindeki etkisini araştırmayı amaçladık. Gereç ve Yöntem:Merkezimizde son üç yıl içinde soğuk ürti-keri tanısı almış 29 hasta retrospektif olarak çalışmaya alındı. Hastaların demografik verileri, hastalık ile ilgili klinik özellikleri ve mevcut test sonuçları toplandı. Tüm olgularda buz testi uygulandı. Merkezimizde standart buz testi sırasında stimülasyon süresi 5 dakika olarak uygulanmaktaydı ve testin negatif bulunması durumunda kademeli olarak önce 10 dakikaya, ardından 20 dakikaya çıkarılmaktaydı. Buz testinde stimülasyon süresinin 20 dakikaya çıkarılmasının testin tanısal değeri üzerindeki etkisini araştırmak amacıyla kronik ürtikeri olan ancak soğuk ile tetiklenme tariflemeyen 20 hastaya stimülasyon süresi 20 dakika olacak şekilde buz testi uygulandı. Bulgular: Çalışmaya soğuk ürtikeri tanısı konan 29 hasta (K/E: 20/9, medyan (min-maks) yaş: 37 (17-57)) alındı. Hastaların hiçbirinde ilişkili olabilecek başka bir hastalık ya da neden saptanmadığından tüm hastalar idyopatik soğuk ürtikeri olarak sınıflandı. Atopi sıklığı %48.0 bulundu. Üç hastada (%10.3) lokalize soğuk ürtikeri saptandı. Sistemik reaksiyonların sıklığı %48.3 (14/29), anafilaksi sıklığı ise %10.3 (3/29) idi. Hastaların %41.4'ünde (12/29) ek olarak diğer ürtiker formları (en sık kronik spontan ürtiker ve semptomatik dermografizm) mevcuttu. Buz testinin duyarlılığı stimülasyon süresi 5 dakika olarak uygulandığında %58.6, stimülasyon süresi kademeli olarak 20 dakikaya uzatıldığında %69.0 bulundu. Kontrol olguların hiçbirinde stimülasyon süresi 20 dakika olacak şekilde uygulanan buz testinde pozitiflik saptanmadı. ABSTRACTObjective: Cold urticaria is an uncommon type of physical urticaria. During the standard ice cube test procedure the stimulation time was five minutes, and if the result was negative it was gradually increased to ten and twenty minutes. To investigate the effect of increasing the stimulation time to twenty minutes on the diagnostic value, the test was performed in twenty patients with chronic urticaria in whom cold was not a triggering factor.Results: Twenty-nine patients (F/M: 20/9, median (min-max) age:37 (17-57)) with cold urticaria were included. La urticaria por frío es una de las cinco causas más frecuentes de urticarias crónicas; está incluida en el grupo de urticarias físicas y puede ocurrir luego de la exposición al frío, ya sea a través de un objeto, el aire o un líquido. Los pacientes pueden presentar síntomas de urticaria, angioedema, dificultad respiratoria e, incluso, anafilaxia al sacar objetos de la nevera, nadar en agua fría o al entrar a cuartos con aire acondicionado. A continuación se presentan cinco pacientes con diagnóstico de esta enfermedad y una breve revisión del tema.Palabras clave: urticaria, angioedema, anafilaxia, antagonistas de histamina, crioglobulinemia, frío. It can occur after exposure to cold, either through solid objects, air or liquids. Patients may have symptoms of urticaria, angioedema, respiratory distress and even anaphilaxis when the skin is exposed to a cold environment, such as handling refrigerated objects, swimming in cold water or entering an air-conditioned room. Caso 1Se trata de un paciente de sexo masculino de 41 años de edad, que consultó por un cuadro clínico de dos meses de evolución consistente en urticaria, angioedema en manos y pies, y dificultad respiratoria cuando estaba en una piscina, que cedió espontáneamente; también había presentado urticaria en un antebrazo al contacto con una botella fría. Tenía antecedentes personales de rinitis, asma y conjuntivitis alérgica. Se le practicó la prueba del cubo de hielo, que fue positiva a los cuatro minutos. Fue tratado con fexofenadina, con mejoría total de los síntomas. Caso 2Se trata de una paciente de sexo femenino de 3 años y 8 meses de edad, que fue llevada a consulta por presentar urticaria y angioedema al contacto con el agua fría. En una ocasión, mientras estaba en una piscina, presentó un episodio de hipotonía generalizada, palidez y pérdida de conciencia, que requirió hospitalización. Con el consumo de bebidas frías presentaba eritema y edema en cuello. Se le practicó la prueba del cubo de hielo, que fue positiva a los seis minutos. Se inició tratamiento con ciproheptadina, consiguiéndose mejoría de la sintomatología. Un mes después del inicio del tratamiento, se hizo una nueva prueba con cubo de hielo, la cual fue negativa a los 10 minutos. La paciente recibió ciproheptadina por seis meses llegando hasta la dosis tope para su peso, pero por persistencia de síntomas asociados al contacto con el frío, se decidió cambiar el tratamiento por cetirizina más montelukast. La paciente ha sido seguida durante cuatro años y en la actualidad todavía presenta urticaria asociada al baño y en piscinas, pero sin afectación respiratoria y de resolución espontánea.
