Combination of endoscopic retrograde cholangiopancreatography and endoscopic ultrasonography‐guided biliary drainage in malignant hilar biliary obstruction

Abstract: Adequate biliary drainage (BD), defined as more than 50% of liver volume drained, is an ideal BD method in patients with advanced and unresectable malignant hilar biliary obstruction (MHBO). Endoscopic retrograde cholangiopancreatography (ERCP) with multi‐segmental BD is technically challenging. ERCP with percutaneous biliary drainage (PTBD) or PTBD alone has cumbersome maintenance of PTBD line and external bag. The utility of EUS‐guided BD (EUS‐BD) has risen significantly over last 5 years mostly in the clini… Show more

“…Also, bilateral drainage is more physiologically sound than unilateral drainage. Various bilateral drainage techniques and SEMS that facilitate bilateral deployment are available . Nonetheless, multiple metal stenting for bilateral or multi‐sectoral drainage is still technically difficult.…”

“…Endoscopic ultrasound‐guided biliary drainage (EUS‐BD) is efficacious in cases with difficulty in biliary drainage using endoscopic retrograded cholangiopancreatography (ERCP) 1–3 . However, the lack of a dedicated EUS‐BD device has led to the adverse event of peritonitis due to bile leakage 3–5 …”

“…In regard to other reinterventions, percutaneous procedures, 25 EUS-guided rendez-vous and EUS-guided biliary drainage have been attempted. 26,27 Failure of reintervention could delay well-timed chemotherapy, undermining the patients' prognosis. In cases of reintervention failure because of the difficulty in performing ordinary ERCP procedures, additional percutaneous or EUS-guided methods could be attempted.…”
