Combination of Sodium Thiosulphate, Cinacalcet, and Paricalcitol in the Treatment of Calciphylaxis with Hyperparathyroidism

Abstract: Calciphylaxis (calcific uremic arteriolopathy) is a severe complication of hemodialysis characterized by subcutaneous calcification of the small arteries and tissue necrosis. Our case report is focused on a woman receiving hemodialysis (HD) with diabetes mellitus for 20 years and severe secondary hyperparathyroidism, who presented painful subcutaneous nodules, skin necrosis and ulcerations. As the treatment of calciphylaxis is mainly empirical and controversial, we decided to administer cinacalcet with parical… Show more

“…3,8 Several strategies aimed at treating and preventing this affliction in the nondialysis population have been reported in the literature. These include early parathyroidectomy, 5,7 sodium thiosulfate, 8,9 vitamin D analogs, 9 bisphosphonates, 10 cinacalcet, 10,11 and hyperbaric oxygen. 12 Unfortunately, none of these maneuvers have been shown to be universally beneficial and the outcome for most patients afflicted with calciphylaxis remains quite poor.…”

“…Cinacalcet has been successfully used in several patients with CUA (22,(51)(52)(53)(54)(55). VDRAs increase the risk of developing hypercalcemia or hyperphosphatemia; however, they have been used successfully in combination with cinacalcet (54,56,57).…”

“…Reports of sodium thiosulfate used to treat CUA first appeared in 2004 (51). There has since been numerous case reports and small series demonstrating effective treatment of CUA with the addition of sodium thiosulfate to the treatment regimen (22,38,(52)(53)(54)(55)(58)(59)(60)(61). Sodium thiosulfate is a chelating agent that has antioxidant activity and increases activity of endothelial nitric oxide synthetase (22); it has been used to treat cyanide toxicity, to reduce toxicity of cisplatin chemotherapy, and to chelate calcium in the treatment of nephrolithiasis (62)(63)(64).…”

“…Furthermore, favourable outcome was recorded in limited reports on CUA using cinacalcet or parathyroidectomy to achieve targeted normalised calcium, phosphate, and parathyroid hormone levels [8, 9]. Yet usefulness of these agents in the form of combination therapy has not been fully explored [10, 11]. The aim of the study was to review all cases of CUA seen in the last five years and determine its clinical course in the setting of multimodality care.…”
