Combined Activity of Sulfamethoxazole, Trimethoprim, and Polymyxin B Against Gram-Negative Bacilli

Abstract: The activity of the three two-drug combinations of sulfamethoxazole (SMX), trimethoprim (TMP), and polymyxin B (PB) against 52 clinical isolates of gram-negative bacilli was studied by a "checkerboard" agar dilution method. The organisms studied included strains of Enterobacter spp., Klebsiella pneumoniae, Serratia marcescens, Providence, Proteus, and Pseudomonas aeruginosa. The majority of these isolates were resistant to at least two of the three agents used in the combined studies and to the most commonly u… Show more

“…3 (Fig. 4) Synergistic effects of STP in vitro against a number of gram-negative bacilli including S. marcescens have been previously recorded (7,14,15,18,19), yet few clinical trials have been forthcoming. In vitro static and cidal tests in our laboratory confirmed the synergistic effect of this three-drug combination against multiply drug-resistant S. marcescens obtained from clinical isolates.…”

“…Furthermore, investigation of other modes of antimicrobial therapy was pursued due to concern over potential acquisition of resistance if amikacin were the only therapeutic agent used in a large epidemic. Having first confirmed in vitro reports of the efficacy of sulfamethoxazole -trimethoprimpolymixin (STP) against Serratia (7,14) in our laboratory, we proceeded to use this combination in the treatment of six acutely ill patients infected with multiply drug-resistant S. marcescens.…”

“…Recently, trimethoprim has been found to act synergistically when combined with rifampin and polymyxin (2,3,6). The combination of an aminoglycoside with trimethoprim alone has been evaluated in only a few previous studies (1,5), and none has assessed the interaction of sulfamethoxazole-trimethoprin, the currently available trimethoprim preparation in the United States, with an aminoglycoside.…”
