Combined anterior and posterior convex epiphysiodesis for progressive congenital scoliosis in children aged ≤ 5 years

Abstract: Six patients aged < or = 5 years with congenital scoliosis due to vertebral malformation were treated by anterior and posterior epiphysiodesis of the convexity. In all cases the pattern of deformity was a kyphoscoliosis. The average age at operation was 3 years 6 months, average follow-up was 4 years 6 months, and average preoperative angles were 42 degrees in the frontal and 36 degrees in the sagittal plane. The fusion included the malformative zone and the superior and inferior adjacent vertebrae. Two patien… Show more

“…VCR enables translation of spinal column and offers the advantage of a controlled manipulation of both the anterior and posterior column with active reconstruction [10,24]. Combined anterior and posterior VCR requires a lengthy operation and has the possibility to harm the anterior vascular and visceral structures [18,32,35]. For severe rigid adult congenital spinal kyphoscoliosis, it is a common practice that more than one VCR has to be done to provide enough correction with substantial risks.…”

“…Keiffer has proposed that fusion in children is beneficial in terms of prevention of deformity progression [15]. Hemiepiphysiodesis and/or convex fusion to cease growth at the opposite site are the most widely used methods.…”

“…Ruf and Harms [13,14] demonstrated a new technique allowing successful hemivertebra excision, notably in small children. Some authors suggest that hemivertebrectomy can be performed at any age, other authors prefer to carry out the intervention during adolescence [11,15].…”

“…The best results were reported with curves that affect five consecutive segments or less [55][56][57]. On the other hand, longer curves have been successfully treated with CGA as well [60].…”
