Combined Anterior Mitral Leaflet Extension and Myectomy in Hypertrophic Obstructive Cardiomyopathy

Abstract: Dr van der Lee et al 1 describe a combined operative procedure for treating patients with obstructive hypertrophic cardiomyopathy. As well as a "standard" subaortic myectomy, they added a glutaraldehydetreated autologous pericardial patch to the anterior leaflet of the mitral valve. The authors applied their combined technique to 29 of the last 32 patients accepted for surgical correction. The mitral valve, specifically the systolic anterior motion of the mitral valve leaflets, is an important component of the … Show more

“…Both techniques are used together with septal myectomy but proponents of extended septal myectomy hold that such an addition to the myectomy procedure is not required. 14 However, because patch extension of the anterior mitral leaflet has shown to be a useful adjunct to myectomy alone and in the light of the high variability in the distribution of myocardial hypertrophy and papillary muscle anatomy, it is doubtful whether myectomy alone is the perfect solution in all patients. 15 In addition, patch extension may be the only intervention in selected cases.…”
