Combined bipedicled latissimus dorsi and groin flap for anterior chest wall reconstruction

Abstract: We provide a case report of reconstruction of a massive chest wall defect after recurrent phyllodes tumour resections. The reconstruction used a bipedicled groin and latissimus dorsi flap, with composite rib autologous reconstruction. The patient successfully recovered from the operation. Our case illustrates the applicability of this flap in the armamentarium of anterior chest wall reconstructive options.

“…Type III conjoined flap with the ninth rib and the lower two digitations of the left serratus anterior muscle was used to reconstruct anterior chest wall defect. 4 In the type IV conjoined flap, double sets of microvascular anastomoses with different recipient vessels are required for restoration of vascularity. The inset of the type IV flap can be more flexible (eg, straight, spiral, folded), with a low risk of congestion on the skin island because of its double venous drainage system.…”

“…Phyllodes tumours are occasionally characterised by very rapid growth; they can present with destruction of surrounding structures and skin by direct invasion or by pressure necrosis. Chest wall involvement requiring excision of ribs and complex reconstruction has been rarely reported 3…”

“…Chest wall involvement by phyllodes is very rarely reported. It often requires multidisciplinary approach with need for chest wall reconstruction 3 4. We are reporting one such case of malignant phyllodes tumour which required huge chest wall resection and reconstruction.…”

“…The first combined flap, reported by Harii et al (1981), was a conjoined latissimus dorsi (LD)-groin flap for upper limb reconstruction. Since then, it was reported only other five times in literature (Boonipat et al, 2019;Chiang et al, 1997;Dzwierzynski et al, 1995;Katsaros et al, 1983;Nicoli et al, 2016). Dzwierzynski et al (1995) were the first to report the conjoined LD-Groin flap as a free flap with double anastomosis and a length of 64 cm.…”
