Combined liver radiation and chemotherapy for palliation of hepatic metastases from colorectal cancer.

Abstract: Standard hepatic irradiation followed by boost radiation to partial liver volumes in combination with chemotherapy is well tolerated without significant acute/late morbidity. Higher radiation doses to partial liver volumes offers improved palliative benefit and may prolong survival without an increase in morbidity.

“…59 Improved local control and survival with higher doses of RT also have been observed by others, with no obvious correlation between dose and potential confounding factors, such as tumor size and performance status. 11,51,59,60 To our knowledge, the largest experience with RT in patients with HCC is from Asia, where sustained, long-term local control and survival have been reported in patients who received treatment with a variety of radiation fractionation schemes with conformal RT techniques. One-year survival rates in those patients ranges from 50% to 95%, and the 5-year survival rate ranges from 9% to 25% after 40 Gy to 60 Gy delivered over 1 to 5 weeks 6,55,[61][62][63] (Table 3).…”

“…Occasionally, when treatment affects a number of symptoms, the individual symptoms are listed and their palliation reported [e.g. neurological symptoms in patients with brain metastases (Kurtz et al, 1981) or general symptoms resulting from hepatic metastases (Mohiuddin et al, 1996)]. …”

“…Fewer than 20% of patients may experience local recurrence 8,20,21 , and even if selection bias is believed to exist in the series data, the results of sbrt remain impressive. Our case also reflects the role played by the new imaging techniques and the importance of cea in managing crc.…”
