Combined modality therapy of esophageal carcinoma

Abstract: One hundred ten patients with epidermoid carcinoma of the esophagus were treated at the Memorial Sloan-Kettering Cancer Center (MSKCC) with combined modality techniques involving preoperative irradiation (RT) and surgery, and with preoperative chemotherapy (CT), surgery, and irradiation. For the 76 patients receiving preoperative RT during the period 1965-1976, the overall resectability rate was 54% with an operative mortality of 12%; long-term survivors (greater than 3 years) were few (7%). For 34 patients re… Show more

“…Our initial study with cisplatin and bleomycin in combined modality therapy suggested that the resection rate of patients receiving preoperative chemotherapy was superior to that seen in a historical control group receiving preoperative radiation therapy (76% versus 5470, P < 0.04), with similar operative mortality ( 1 1% and 12% respectively). 16 Using the three drug combination of DVB, the 82% resection rate seen to date supports the observation that preoperative chemotherapy is at least as good as preoperative irradiation, and, as noted above, may offer superior resection rates. Systemic chemotherapy has the theoretical advantage, in comparison to radiation, of allowing treatment of both the primary tumor and of metastases outside the local area.…”

“…Gisselbrecht et al 9) have reported that the objective response rate in 21 patients receiving combination chemotherapy with 5-FU, adriamycin, and cisplatin was 33%, but the median length of survival was only 8 months. Various combinations of treatment modalities, such as surgery with pre- or postoperative radiotherapy, 5 , 10 , 11) preoperative chemotherapy followed by surgery, 12) and combinations of radiotherapy and chemotherapy, 8 , 12) have been tried to improve the response rate and longterm survival, demonstrating that combined modality treatment has not been always advantageous compared with single modality treatment. 10) Some reports have shown that the combination of chemotherapy with radiotherapy was much more efficient than single modality with chemotherapy or radiotherapy for inoperable esophageal cancer.…”

“…We were prompted to utilize only DB chemotherapy preoperatively because we observed it to have activity in patients with advanced disease, and because our experience with preoperative radiation therapy at this institution showed a 5-year survival rate of 5%. 8 We reasoned that preoperative treatment with chemotherapy, unlike preoperative radiation therapy, had the potential advantage of destroying tumor at the primary site as well as in occult metastatic sites.…”
