Combined toric intraocular lens implantation and relaxing incisions to reduce high preexisting astigmatism

Abstract: Implanting a toric IOL in patients with high astigmatism reduced the amount of incisional surgery required. Combining techniques can correct all or most of even very high astigmatism (>5.00 D) while avoiding induced corneal irregularities.

“…8,15 Decentration is the main complication (10 to 15% of patients). 8 Excimer laser can be used postoperatively to correct preexisting astigmatism in cataract patients [16][17][18] but it is expensive and requires a refractive surgeon. Also, complications such as loss of BCVA, a decentered zone, flap complications, difficult night vision etc.…”

“…"Limbal relaxing incisions" (LRIs) during phacoemulsification in addition to the clear corneal stab incision of phacoemulsification is one option. Other options include placement of phacoemulsification incision on the steep corneal meridian, excimer laser ablation for postoperative astigmatism and toric intraocular lens implantation [2][3][4][5][6][7].…”
