Common and uncommon lesions of the vulva and vagina on magnetic resonance imaging: correlations with pathological findings

Abstract: Vulvar and vaginal lesions representing a wide spectrum of diseases in female lower genital tract diseases make up a small part of all gynecological etiologies. Many of them are rare etiologies and are reported in case-reports studies. Translabial and transperineal ultrasound are modalities of choice for the first evaluation of perineal lesions. MRI is usually performed to determine the etiology of the lesions and stage. Benign lesions of the vulva and vagina usually manifest as simple cystic (vestibular cyst … Show more

“…The presence of atypical cells, hypercellularity, and numerous mitoses indicates malignancy on histopathological examination. 6,7,15 In our case, the MRI results showed a mass with an isointense signal on T1W1 and T2W1 with relatively well-defined borders, seemingly surrounded by fat, no connection to the surrounding bones, and a size of 5.85x6.03x4.04 cm, located in the right anterolateral wall of the vagina. This mass exhibited mild inhomogeneous enhancement on T1 with contrast.…”

“…In any case, vaginal fibroepithelial polyps should be differentiated from malignant neoplasms of the vagina, such as sarcoma botryoides, rhabdomyosarcoma, and mixed mesodermal tumors [12]. Furthermore, vaginal leiomyomas, although extremely rare, should be included in the differential diagnosis of vaginal fibroepithelial polyps [13].…”

“…Although leiomyomas are the most frequently benign compact neoplasm of the vulva, they are still relatively uncommon. Vulvar leiomyomas represent only 0.03% of all gynecological neoplasms and 0.07% of all vulvar tumors [ 2 ].…”
