Abstract: Health equity drives quality care. Few reliable metrics that capture patients’ perceptions of health equity exist. We report on the development of a patient-centered metric for health systems change in central Brooklyn, which stands out as an outlier in New York City with a disproportionate burden of poverty, disease, and death.
