Comorbidities and Risk of Complications After Surgery for Esophageal Cancer: A Nationwide Cohort Study in Sweden

Abstract: Cardiac disease and a Charlson comorbidity index score ≥2 seem to increase the risk of severe and early post-operative complications in patients with esophageal cancer, while hypertension, pulmonary disorders, diabetes, and obesity do not. These findings should be considered in the clinical decision-making for improved selection of patients for surgery.

“…Lifestyles play a distinct role in the occurrence of the disease, with reports highlighting factors such as alcohol consumption, smoking, and genetic variants leading to an increased risk of EC (4,5). Existing literature has identified surgery as one of the most conventional therapeutic avenues for patients with early-stage EC, whereas perioperative chemotherapy or chemoradiotherapy is often applied for patients with locally advanced-stage EC (6)(7)(8). However, the majority of treatment protocols are still accompanied by an unsatisfactory overall 5-yr survival rate, owing to factors such as late diagnosis, tumor invasion, and metastasis as well as the side effects and resistance encountered by certain patients in confined drugs (9)(10)(11).…”

“…The Charlson co-morbidity index was calculated excluding points for age and solid tumour, as described in the Swedish retrospective study by Backemar et al which assessed the use of this score in oesophagectomy patients. 17 The American Society of Anesthesiologists (ASA) score was determined using the 2014 guideline. 18 The study met the New Zealand National Ethics Committee Guidelines definition of an audit and therefore did not require ethics committee review.…”
