Comparable Outcomes Between Native and Periprosthetic Fractures of the Distal Femur

Abstract: Despite the rising prevalence of arthroplasty and aging population, limited data exist regarding differences in periprosthetic fracture clinical outcomes compared with native counterparts. This study compares differences in hospital treatment, morbidity, and mortality associated with periprosthetic distal femur fractures at an urban level 1 trauma center. We retrospectively reviewed all adult AO/OTA type 33 fractures (526) that presented to our institution between 2009 and 2018. In total, 54 native and 54 peri… Show more

“…More than one-third were discharged to step-down care, 16% suffered from a medical complication, and 8% were readmitted to hospital, with a 6.4% in-hospital mortality rate. Our results demonstrate that the mortality rates seen in periprosthetic fractures around the knee are similar to those seen in fragility fractures of the distal femur, 6,8 and similar to or higher than those for neck of femur fracture. 3,23 Despite this, we still lack a national PPF database as well as standardized national management guidelines for these complex injuries.…”

“…[3][4][5][6][7] These mortality rates are similar to that seen in native distal femur fragility fractures. 8 Abram et al 9 concluded that revision arthroplasty for a PPF of the knee was associated with a significantly higher mortality rate compared to revision surgery for a planned indication. Between 30% to 50% of patients may die, sustain a complication, or fail to regain their mobility after sustaining a PPF around the knee.…”

“…The reported incidence of these fractures ranges between 0.3% and 5.5% (3). The patient population of periprosthetic fractures and non-periprosthetic fractures after low-energy trauma generally have similar demographic characteristics (4,5).…”
