Comparative Analysis of 3 Different Construct Systems for Single-level Anterior Cervical Discectomy and Fusion

Abstract: The trend of excellent radiologic outcome was observed for IP≥CP>CA. Plating may play a key role in the support of anterior height. As a result, plating prevents segmental kyphosis and subsidence and promotes bone fusion. Although the overall clinical outcomes were not different among the 3 groups, except for arm pain, more favorable trends regarding clinical outcome were observed for CP≥IP>CA.

“…At present, various cages are widely used for interbody fusion to avoid morbidity at the donor site and complications arising from the use of autologous iliac bone. Surgeons also prefer to add an anterior plate to enhance rigidity of fixation especially in multi-level cases (11,23,24). Although the design of the current anterior plate has been improved over the years, the complications related to the plates are still not rare, and include screw loosening or breakage, plate breakage, and screw pullout with or without migration into the gastrointestinal tract (15,25).…”

“…Another possible mechanism for high postoperative dysphagia rate following ACDF with plate may be the additional traction and more time needed to control the angle of the screws and length of the plate to place the plate correctly (11,14,15). Increased pressure on the esophagus during implantation of the plate and direct stimulus of esophagus may contribute to the dysphagia (5, 31).…”

“…Moreover numerous complications results from autologous tricortical iliac crest bone graft including protrusion of bone block, breaking of bone graft, subsidence, collapsing, angular deformation, pseudo arthrosis 7 . To overcome these complications, it is replaced by standalone case packed with autologous iliac crest bone graft, however, it causes a number of donor site morbidities including infection, pain and numbness 8,9 . To avoid these complications, bone substitutes or allobone have been used.…”

“…Typical symptoms are neck pain radiating into the arm, possible loss of motor function and/or sensory loss [1]. The most common surgical treatment is discectomy and fusion [2] either as stand-alone implant surgery, or with the addition of anterior plating [3]. Anterior cervical discectomy is one of the most frequently performed spinal procedures.…”
