Comparative Clinical Characteristics of Rheumatic Heart Disease Patients Undergoing Surgical Valve Replacement

Abstract: Background To assess the prevalence patterns of isolated/mixed rheumatic valvular lesions and associated risk factors among rheumatic heart disease (RHD) patients undergoing surgical valve replacement. Methods An analytical cross-sectional design was used. Purposive sampling was used to select 87 RHD patients who underwent a first-time valve replacement for mitral, aortic, or both valves between April 1 and October 20, 2016, at Punjab Institute of Cardiology, Lahore, Pakistan. … Show more

“…This result was in line with several results found by various researchers (Balakrishnan, 2015;Brewer & Kerslake, 2015;Francisco et al, 2015;Kokkinos & Antoniadou, 2019) indicating that a correlation exists between CP and CV. Contrary to this finding, the result of a study carried out in Pakistan by Shahbaz Butt et al (2019) revealed a significant positive correlation between CB and CV. The point to indicate the correlation between CP and CV was to figure out how CP would lead to CV.…”

“…Butt and colleagues conducted a cross sectional survey of the patients with rheumatic heart disease undergoing surgery. 8 The mean age of the patients in their study was 32.79 ± 13.06 years demonstrating the younger age of the patients with rheumatic heart disease in this part of the world. This is in contrast to the studies published in the western literature.…”

“…Mean LAVi values for isolated mitral regurgitation and isolated mitral stenosis were 193 and 110 ml/m², respectively, whereas only a mild increase of LAVi (51 and 42 ml/m², respectively) was found in isolated aortic regurgitation and isolated aortic stenosis. Butt et al 16 reported that only 36% of patients with significant aortic regurgitation or stenosis who underwent surgery were observed with LA enlargement. Increased LV end-diastolic pressure in these patients may be responsible for the increased LA size.²² A significant proportion of RHD patients had also reduced RV contractility, especially in patients with mitral valve involvement (isolated mitral lesion or combined with aortic valve lesions compared with aortic valve lesion only, p<0.001).…”
