13-cis retinoic acid (13-cis RA; also known as isotretinoin) is the most potent agent available for treatment of acne. It is known that the drug induces apoptosis in cells cultured from human sebaceous glands, but its mechanism of action has not been determined. In this study, skin biopsies were taken from 7 patients with acne prior to and at 1 week of treatment with 13-cis RA. TUNEL staining confirmed that 13-cis RA induced apoptosis in sebaceous glands. Transcriptional profiling of patient skin and cultured human sebaceous gland cells (SEB-1 sebocytes) indicated that lipocalin 2 was among the genes most highly upregulated by 13-cis RA. Lipocalin 2 encodes neutrophil gelatinase-associated lipocalin (NGAL), which functions in innate immune defense and induces apoptosis of murine B lymphocytes. Increased immunolocalization of NGAL was noted in patients' sebaceous glands following treatment with 13-cis RA, and recombinant NGAL induced apoptosis in SEB-1 sebocytes. Furthermore, apoptosis in response to 13-cis RA was inhibited in the presence of siRNA to lipocalin 2. These data indicate that NGAL mediates the apoptotic effect of 13-cis RA and suggest that agents that selectively induce NGAL expression in sebaceous glands might represent therapeutic alternatives to the use of 13-cis RA to treat individuals with acne.

From the first reliable descriptions of acne in the early 19th century, dermatologists recognized it as a disease of the pilosebaceous follicle. Until the middle of the 20th century, they hypothesized that seborrhoea, follicular keratosis and microorganisms could be individually responsible for the acne lesions. Inflammation was only regarded as the final and inescapable step of the acne process. Although the importance of these factors has been reevaluated, recent works still regarded them as mandatory. In the 1970s, the onset of isotretinoin dramatically improved acne management. It also provided great opportunities for a better understanding of the pathogenic factors of acne. This study analyzes their genesis and development from the seminal contributions until recent advances.

Oral isotretinoin is a highly successful therapy for severe, refractory inflammatory acne vulgaris (AV) that is indicated especially for patients with multiple nodulocystic lesions. After over 30 years of clinical use, it is apparent that the majority of patients experience clearing of AV with prolonged remission after a course of oral isotretinoin therapy is completed. Multiple reports have shown that recurrence of AV occurs in some patients and is more likely in those who had not achieved adequate cumulative exposure to a total dose of drug, which has been defined as 120 -150-mg/kg. As the pharmacokinetic profile of orally administered isotretinoin shows that gastrointestinal absorption is highly dependent on administration with a high caloriehigh fat meal, it is has been considered that patients who ingest a conventional formulation of oral isotretinoin without an adequate meal or without food at all may be predisposed to a higher rate and earlier onset of recurrence of AV as their cumulative systemic exposure is effectively reduced by up to 60 %. A new formulation of oral isotretinoin uses a technology where isotretinoin is pre-solubilized in a lipid matrix so absorption is not markedly diminished if the patient does not ingest the drug with an adequate meal. As the data analysis on recurrence of AV in patients treated with conventional formulations of oral isotretinoin are retrospective, additional research and vigilant observation are needed to determine if the new formulation will provide better long term outcomes.
