Introduction: During endodontic treatment, the smear layer can reduce disinfectant efficacy. The aim of this study was to evaluate the antimicrobial efficacy of Er:YAG laser radiation and its combination with NaOCl and QMix against Enterococcus faecalis and Candida albicans and their comparison with conventional irrigation with only NaOCl and QMix. Methods: Two hundred extracted single-rooted teeth after root canal preparation were divided into two groups and inoculated with E. faecalis and C. albicans. According to the treatment method, all samples were divided into five treatment groups: (1) Er:YAG laser, (2) NaOCl, (3) QMix, (4) Er:YAG laser plus NaOCl, and (5) Er:YAG laser plus QMix. After 24 hours of agar plate cultivation, cell viability was recorded with a flow cytometer. Results: All treatment modalities showed efficiency in the reduction of microbial cells. For laser treatment alone after exposure for 90 seconds, significantly fewer non-death cells were seen, compared to 30-second treatment. For both timings (30 and 90 seconds), irrigation with NaOCl or QMix after laser application resulted in significantly fewer vital cells of E. faecalis and C. albicans, compared with laser treatment alone (P<0.001). The samples treated with only NaOCl showed a significantly higher percentage of vital E. faecalis and Candida albicans cells, compared to the samples treated with only QMix (P<0.001). Conclusion: Higher Er: YAG laser exposure time (90 seconds) after its combination with QMix and NaOCl improves the efficacy in the reduction of E. faecalis and C. albicans from the root canal, compared to 30-second laser exposure time and conventional irrigation methods with NaOCl and QMix.

Introduction: Developing regenerative endodontic treatment (RET) is an exciting approach to managing immature permanent teeth with pulp necrosis. RET is usually performed in two clinical steps: disinfection (first step) and tissue engineering (second step). Recently, laser therapy has entered the field of RET. This study aimed to provide an overview of the literature that employed laser therapy for root regeneration. Methods: A comprehensive search was performed on four databases, including PubMed, Web of Science, Scopus, and Google Scholar. The searched keywords were laser, regenerative endodontics, immature permanent teeth, and dental pulp necrosis, and related English-published articles were included up to October 2023. Results: Thirteen studies utilized a laser for RET. In the first step of RET, both high-power and low-level lasers (through photodynamic therapy [PDT]) may be applied for canal disinfection. In contrast, regenerative procedures in the second step of RET are just accelerated by low-power lasers (biostimulation). The literature does not support the benefit of laser-assisted irrigation in improving the clinical success of RET. There is some evidence that laser-assisted disinfection with a diode laser may provide comparable results to triple antibiotic paste in reducing bacterial counts in root canals while providing slightly better clinical and radiographic outcomes. PDT may be an effective and suitable adjunct to conventional disinfection methods in immature, necrotic teeth. Conclusion: Low-power lasers may be beneficial tools for improving the results of regenerative endodontics through chemical disinfection in the first step (PDT) or by biostimulation in the second step of RET.

Background Surface tension and contact angle properties, which play a crucial role in determining the effectiveness of irrigation solutions in penetrating dentin surfaces and dentin tubules, are highly important for the development of new irrigation solutions and their preferences. The aim of the current study was to compare the surface tension and contact angle properties of different irrigation solutions used in endodontics, both on the dentin surface and within dentin tubules. Methods In this study, the contact angles and surface tensions of 5.25% sodium hypochlorite (NaOCl), 17% ethylenediaminetetraacetic acid (EDTA), 2% chlorhexidine (CHX), 5% boric acid (BA), 0.02% hypochlorous acid (HOCl), 0.2% chlorine dioxide (ClO2), Biopure MTAD, QMix solutions, and distilled water (control group) were measured. Measurements were conducted using a goniometer device (Attension Theta Lite Tensiometer, Biolin Scientific, USA), employing the sessile drop method for contact angle measurements on pre-prepared dentin surfaces, and the pendant drop method for surface tension. Results Contact angle measurements revealed no statistically significant differences between the contact angle values of MTAD, ClO2, and CHX or between NaOCl, QMix, BA, and HOCl (p > 0.05). However, EDTA exhibited a significantly greater contact angle than did MTAD, ClO2, CHX, NaOCl, QMix, BA, and HOCl (p < 0.05). Furthermore, the contact angle of dentin with distilled water was greater than that with all other solutions tested (p < 0.05). Surface tension measurements revealed that the surface tension values of QMix and MTAD were statistically similar (p > 0.05). CHX exhibited lower surface tension than distilled water and HOCl (p < 0.05), and it also had lower surface tension than ClO2, NaOCl, and BA (p < 0.05). Additionally, the surface tension of the samples treated with EDTA was greater than that of all other solutions tested (p < 0.05). Conclusion The direct linear relationship between the surface tension of liquids and contact angles on different surfaces may not always hold true, and these values should be considered independently for each solution on various surfaces. Considering the contact angles and surface tension properties of irrigation solutions with root canal dentin, it can be suggested for clinical use that ClO2 could be recommended over NaOCl, and similarly, BA could be recommended over EDTA.
