Comparative Outcomes in Functional Rhinoplasty With Open vs Endonasal Spreader Graft Placement

Abstract: Spreader grafts can be placed through an endonasal or open approach with similar outcomes in a standardized measure. Open rhinoplasty is associated with prolonged operative times and therefore increased operative costs. In properly selected patients, endonasal spreader graft placement may lead to significant cost savings when open rhinoplasty is not otherwise indicated.

“…When comparing open with endonasal spreader graft placement, the open approach was associated with longer operative times with equivalent NOSE score improvement, although no objective measures were included in the study. 7 The authors of that study estimated that undergoing an endonasal technique would lead to a theoretical mean cost difference of $1,912.85 and an overall charged difference of $17,842.10 to the patient/insurance company due to decreased operative times. Multiple studies have further confirmed that the endonasal approach is effective at improving symptoms associated with INV dysfunction, and thus, should be considered a reliable option.…”

“…According to Yoo and Jen, the combination of intercartilaginous and transfixing incisions made an excellent exposure of the nasal dorsum. [11][12][13][14][15][16][17] Toriumi and Ries support the idea of unilateral grafts, positioning the spreader graft on the concave side of the Cshaped septal deviation to restore the function and harmonize the aesthetic line. Likewise, in linear deviations of the nasal pyramid, it is suggested placing the spreader graft on the opposite side of the deviation.…”

“…13 However, in contrast, Hafezi et al highlight the importance of bilateral grafts to neutralize the cartilage memory with greater firmness. 8,[14][15][16][17][18] In our study, based on the idea of bilateral grafts according to Hafezi et al's description, we included patients who were treated with bilateral placement. To perform the correct fixation and stabilization of the spreader graft, we consider necessary to separate the septolateral junction, but keeping safe the bilateral mucoperiosteum.…”
