Comparing Methods for Targeted Axillary Dissection in Breast Cancer Patients: A Nationwide, Retrospective Study

Abstract: Background Several techniques exist for performing targeted axillary dissection (TAD) after neoadjuvant chemotherapy with the removal of the sentinel node and a marked metastatic lymph node (LN). Two-step methods include coil-marking of the metastatic LN at diagnosis and re-marking with an intraoperatively identifiable marker before surgery. Because nondetection of the marked lymph node (MLN) warrants axillary clearance and many patients achieve axillary pathological complete response (ax-pCR), t… Show more

“…In 17 studies 17 , 19 , 27 , 30–43 , on the day of surgery a wire was placed after NST in the clipped TLN. In 12 studies 17 , 21 , 41 , 42 , 44–51 , the clipped TLN was marked with a 125 I seed, either before or after NST, and in 5 studies 52–56 , a form of 99m Tc was used to localize and excise the clipped TLN. In three of five studies 52–54 , the clipped TLN was injected with 99m Tc-labelled macroaggregated albumin under ultrasound guidance 1 day before surgery.…”
