Comparing the utility of 30- and 60-minute cortisol levels after the standard short synacthen test to determine adrenal insufficiency

Abstract: Short Synacthen test (SST) involves measuring the baseline, 30-, and 60-minute serum cortisol levels, after injecting 250 μg of synthetic adrenocorticotropic hormone or Synacthen (ACTH). This study aimed to review the current clinical practice of performing SST to establish a standardized test protocol and to additionally test the hypothesis regarding performing the 60-minute cortisol test alone and the dependence of overall SST result on baseline cortisol level. Patients >14 years who underwent SST… Show more

“…Furthermore, if patient management was to be based solely on the 30-minutes sample, they would at least have to undergo further tests or be commenced on unnecessary, long-term steroid replacement therapy. This conclusion was supported in a recent paper by Butt et al 8 who looked at 849 people undergoing SSTs and reported that 9.5% of patients had a suboptimal response at 30 minutes, but reached the threshold value at 60 minutes. There is also the possibility that the 60-minute serum cortisol level may actually be more representative of physiological adrenocortical function than the 30-minute serum cortisol.…”

“… 17 We acknowledge that this strategy may result in between 7% and12% false positive results (which normalise in the 60 minute post Synacthen sample in the 60 minute SST) but have done so in line with local practice and pragmatic reasons. 18 …”
