Comparison of 68Ga-FAPI-04 and fluorine-18-fluorodeoxyglucose PET/computed tomography in the detection of ovarian malignancies

Abstract: Background Currently, fluorine-18-fluorodeoxyglucose ( 18 F-FDG) is the most frequently used diagnostical radiotracer for PET/computed tomography (PET/CT) in ovarian malignancies. However, 18 F-FDG has some limitations. The fibroblast activation protein inhibitor (FAPI) previously demonstrated highly promising results in studies on various tumor entities and 68 Ga-labeled FAPI presents a promising alternative to 18 F-FDG. This study aimed to compare the performance of 68 Ga-FAPI and 18 F-FDG PET/CT for imaging… Show more

“…In addition, a retrospective study by Zheng et al. ( 57 ) in 27 patients with pathologically confirmed ovarian cancer showed that compared with 18 F-FDG, 68 Ga-FAPI PET/CT could more accurately differentiate between the different types of malignant ovarian tumors, including high-grade serous carcinomas (HGSC), low-grade serous carcinomas (LGSC), and ovarian mucinous carcinoma. Moreover, Zheng et al.…”

“…Moreover, Zheng et al. ( 57 ) found that 68 Ga-FAPI PET/CT was more sensitive than 18 F-FDG PET/CT in the detection of primary ovarian tumors (14/14 (100%) versus 11/14 (78%)), lymph node metastasis (75/75 (100%) versus 60/75 (80%)), and peritoneal and pleural metastasis (9/9 (100%) versus 5/9 (56%). These findings suggest that 68 Ga-FAPI PET/CT could supplement the diagnostic information provided by 18 F-FDG PET/CT.…”

“…Zheng et al retrospectively compared [ 68 Ga]Ga-FAPI-04 and FDG PET/CT in 21 patients with suspected (n = 11) or already diagnosed (n = 10) primary ovarian malignancy. FAPI was more sensitive than FDG PET/CT for the detection of primary tumors (100% vs. 78%), nodal metastases (100% vs. 80%), and peritoneal/pleural involvement (100% vs. 56%), leading to upstaging of disease in 19% (4/21) of the patients [118]. Figure 8 shows an example of FAPI PET/CT in a patient with ovarian malignancy.…”

“…The role of [ 68 Ga]Ga-FAPI is still being explored as scores of papers are being published demonstrating the use of this tracer in various oncologic and non-oncologic applications [16]. It was found to have comparable performance to [ 18 F]F-FDG PET in patients with head and neck carcinoma and soft tissue sarcomas [17,18], while it outperformed [ 18 F]F-FDG PET in abdominal and pelvic malignancies (gastric, ovarian, hepatocellular, and bladder) [19][20][21][22][23]. It also fared well in IGG4 related diseases [24].…”

“…It also fared well in IGG4 related diseases [24]. In gynecological malignancies, [ 68 Ga]Ga-FAPI, displayed higher accuracy for detecting malignant lesions as is seen in ovarian cancer with carcinomatosis [19]. In a study that evaluated the use of [ 68 Ga]Ga-FAPI in cervical cancer, the authors correlated their imaging findings with histopathological results from surgical specimen [25].…”
