Comparison of a propofol target‐controlled infusion and inhalational sevoflurane for fibreoptic intubation under spontaneous ventilation

Abstract: Sevoflurane provides good fibreoptic intubation conditions to spontaneously breathing patients without any hypoxemic episodes such as those observed with propofol.

“…Propofol and sevoflurane have been compared in this setting with little group differences except that patients receiving propofol had more frequent episodes of hypoxemia. 48 Easy and titratable anesthesia was provided in both groups. Similar findings have been reported in patients with anticipated difficult airways using sevoflurane 49 and propofol 50 with the aid of pressure support ventilation.…”

“…Dose-response curve for desfl urane plotted from the probit regression analysis of the individual end-tidal concentrations and the respective probabilities of successful LMA insertion. The 95% confi dence interval (CI) of the EC 50 for desfl urane for successful LMA insertion in nonparalyzed patients co-induced with 50% nitrous oxide and fentanyl (1.5 μg·kg −1 ) was determined as 3.13-3.90 * P < 0.05 compared with mean blood pressure and heart rate at 1 min after LMA insertion attempt Values are expressed as means ± SD or number of patients ventilation, and allows immediate and automatic reduction or cessation of delivery on poor ventilation or apnea, thereby reducing the risk of hypoxemic episodes during airway intervention [15]. In addition, propofol induces signifi cant decreases in mean arterial pressure in patients with little cardiovascular reserve.…”

“…Similar results have been found in our study with fibreoptic tracheal intubation. Bonnin et al [27] reported 5 episodes of desaturation (SpO 2 under 90%) occurred during fibreoptic intubation in Propofol group compared with none in Sevoflurane group. Episodes of desaturation in our study might be due to the use of Propofol in our study.…”
