Comparison of Blade Temperature Dynamics after Activation of Harmonic Ace Scalpel and the Ultracision Harmonic Scalpel LCS-K5

Abstract: The newer-generation Harmonic Ace may be potentially more effective in tissue dissection, but is associated with greater increases in peak temperatures and longer blade hyperthermia periods compared to the older-generation Ultracision Harmonic Scalpel LCS-K5. The difference is greater after longer periods of activation, and this may have more potential for collateral damage, particularly to nerves and bowel.

“…These devices allowed a reduction in operative time and a decrease in postoperative complications. [1][2][3][4][5][6] Surgical devices are regularly modified in an effort to improve results. In 2011, the Food and Drug Administration approved the Small Jaw, a new generation of the LigaSure (LS) (Covidien, Mansfield, MA) for use in thyroid surgery.…”

“…The main advantages of ultrasonic coagulating/dissecting systems compared with a standard electrosurgical device are manifested by minimal lateral thermal tissue damage, less smoke formation, no neuromuscular stimulation, and no electrical energy through the patient[1,2]. This new technology is extensively used in laparoscopic surgery and several fields of minimally invasive surgery [3-5].…”

“…17 The articulating jaw compresses tissue against the vibrating shaft to impart the tissue effects. 18 The Harmonic ACE is associated with greater increases in peak temperatures at the instrument tip compared with the older model, 18 and so results from studies performed using the older model cannot be directly extrapolated to the current model. Vessel-sealing tissue effects are essentially the same as those obtained with advanced bipolar electrosurgery, namely, desiccation and coagulation with resultant coaptation.…”
