Objective: Laparoscopic hepatectomy is a technique that has gained ground over the years, and its benefits have been similar to or better than those of the open hepatectomy technique. We developed this study to demonstrate, based on our 5-year experience, the advantages of laparoscopic surgery in liver surgery. Materials and methods: Through a retrospective, comparative study, a population of 15 people, women and men, with a malignant or benign diagnosis of liver injury, who underwent open or laparoscopic surgery, was analyzed to demonstrate the advantages and disadvantages of one or another technique regarding surgery time, post-operative recovery, and the presence or absence of free margins. Results: Through the statistical analysis based on the data obtained from the patients, no significant difference was found in the reduction of laparoscopic versus open surgery time. One case presented positive margins for malignancy by histopathology analysis approached by laparoscopic technique. However, statistically, it did not show a difference against the open technique. Conclusion: Laparoscopic and open liver surgery showed the same efficacy and safety. As a first approach, there is a tendency for the laparoscopic technique over the open one. Technological advances and techniques are expected to continue to grow as rapidly as they have in recent years.

In a meta‐analysis, we assessed the impact of different surgical approaches on the outcome of hepatectomy with hepatocellular carcinoma. Four databases, including PubMed, Embase, Cochrane Library, and the Web of Science, have been critically reviewed through the full literature through June 2023. Eleven related trials were examined once they had met the trial's classification and exclusion criteria, as well as the assessment of the quality. A random effects approach was applied to analysis of operative organ infections, and a fixed‐effect model was applied to determine the 95% CI and OR. Analysis of the data was done with RevMan 5.3. Our findings indicated that patients undergoing minimally invasive liver cancer surgery had significantly lower risks of surgical organ infection (OR, 0.35; 95% CI, 0.16–0.77; p = 0.009) and wound infection (OR, 0.19; 95% CI, 0.13–0.28; p < 0.001) compared to those undergoing open surgery. There was no heterogeneity observed between the two groups (I2 = 0) in wound infection. Nevertheless, because of the limited number of randomised controlled trials in this meta‐analysis, care should be taken and carefully considered in the treatment of these values. Further high‐quality studies involving a large number of samples are needed to validate and reinforce the results.

RESUMOO câncer hepático atinge mais de 800.000 novas pessoas a cada ano, levando a 700.000 óbitos e sobrevida estimada em 5 anos. Ele se caracteriza por ter origem em células hepáticas (hepatócitos, epitélio dos ductos hepáticos ou tecido mesenquimal hepático) ou ainda atingir o fígado de forma metastática, com origem em outros órgãos. Atualmente, são diversas as técnicas para tratamento dessa moléstia, mas apenas uma técnica apresenta um potencial curativo: a hepatectomia. Essa técnica é utilizada no tratamento de tumores malignos, metastáticos ou não, assim como nas lesões pré-malignas. O objetivo deste trabalho é fazer uma revisão de literatura a cerca de duas diferentes técnicas para realização de uma hepatectomia (aberta ou videolaparoscópica) a fim de destacar suas diferenças, complicações e indicações. Analisadas as técnicas em questão, poder-se-á identificar, em cada cenário, as principais características de cada uma delas, e ainda definir a melhor indicação a depender dos diversos pacientes que serão encontrados.
