Comparison of exercise capacity in COPD and other etiologies of chronic respiratory failure requiring non-invasive mechanical ventilation at home: retrospective analysis of 1-year follow-up

Abstract: IntroductionThe objective of this study was to compare the change in 6-minute walking distance (6MWD) in 1 year as an indicator of exercise capacity among patients undergoing home non-invasive mechanical ventilation (NIMV) due to chronic hypercapnic respiratory failure (CHRF) caused by different etiologies.MethodsThis retrospective cohort study was conducted in a tertiary pulmonary disease hospital in patients who had completed 1-year follow-up under home NIMV because of CHRF with different etiologies (ie, chr… Show more

“…Short-term rates of hospitalization were reduced by PAP, but the event rate was low and data were reported in only one RCT ( 19 ). Longer-term data suggest high rates of hospitalization in patients with OHS on PAP but without a comparison group (data from five observational studies: 10% at 3 mo to 49% at 5 yr) ( 20 , 22 , 65 , 70 , 74 ).…”

“…The combination of home NIV and exercise training has been demonstrated to deliver a larger improvement in blood gases than either intervention alone (63). Home NIV resulted in a greater improvement in 6MWD when coupled with pulmonary rehabilitation than rehabilitation alone both in controlled and uncontrolled studies (64)(65)(66). However, systematic reviews and meta-analyses are unable to recommend the addition of NIV during exercise therapy due to the lack of supporting evidence (67)(68)(69).…”

“…However, we know that there are studies demonstrating an enhanced exercise capacity with long-term NIV therapy in COPD patients [7,8]. We think that the 6-min walk test, which indicates a patient's exercise capacity, needs to be performed in addition to the SRI questionnaire while evaluating beneficial effects of NIV.…”
