Comparison of Five-Day Cefdinir Treatment with Ten-Day Low Dose Amoxicillin/Clavulanate Treatment for Acute Otitis Media

Abstract: For children with nonrefractory AOM, based only on clinical endpoints, 5 days of therapy with cefdinir 14 mg/kg divided twice daily was comparable overall with 10 days of therapy with low dose amoxicillin/clavulanate 45/6.4 mg/kg divided twice daily.

“…Although there have been prior studies of amox/clav and cefdinir as treatment of AOM 2,3 this is the first study, to our knowledge, where amox/clav HD for 10 days has been compared to cefdinir for 5 days, both approved regimens for AOM in children. The superior outcome for 10 days amox/clav HD over 5 days of cefdinir could be predicted by in vitro testing of the drugs against the dominant otopathogens of AOM 4 .…”

“…[41][42][43][44][45][46][47][48][49] In all of these reports, clinical efficacy was judged to be around 80%, which is not significant. However, it has been reported that within a confidence limit of 95%, oral cephems, macrolides, and CTRX are slightly less clinically effective than AMPC and CVA/AMPC, and depending on the method of use they are clearly clinically less effective.…”

“…As mentioned before, many comparative clinical studies involved doses of AMPC or CVA/AMPC for a period of 10 days. 31,[64][65][66][67][68][69] This could be based on clinical experience of recurrence and relapse. Pichichero et al 70 observed 3 groups of AOM patients receiving doses of antimicrobial agents for 5, 7, and 10 days, respectively.…”
