Comparison of Netilmicin and Amikacin in Treatment of Complicated Urinary Tract Infections

Abstract: Netilmicin and amikacin, two recently developed aminoglycosides, were compared in a prospective, randomized study of 57 male patients with complicated urinary tract infections. Both drugs were administered intramuscularly every 12 h for 7 to 10 days, netilmicin at 2 mg/kg and amikacin at 7.5 mg/kg. The two groups were comparable as to infecting bacteria and underlying pathology of the urinary tract. No patients had indwelling catheters. All microorganisms isolated were sensitive to both antibiotics. A total of… Show more

“…However, comparable in vitro data and response to treatment suggest that these two aminoglycosides have similar efficacy with susceptible pathogens. Response rates to both drugs were similar to those noted previously (2,5,11,12,16,18,29,30). Most of the patients who had infections with gentamicin-resistant pathogens responded, but the small number of patients makes comparison between the two drugs difficult.…”

“…Initial serum levels were predictable in only a small majority of our patients, although levels were less variable than gentamicin in another study (26). Mean peak serum levels were similar to those previously reported using comparable doses (16,22,29,30), although some investigators (16,29) did not adjust initial doses for reduced creatinine clearances. Because initial levels may be unpredictable, peak and valley levels should be obtained early in netilmicin therapy, as with other aminoglycosides.…”

“…In no instance was a moxalactamhydrolyzing enzyme detected. (3,8). Moreover, moxalactam cured 4 of the 10 patients infected with gentamicin-resistant, moxalactam-susceptible bacteria.…”
