Postanesthetic shivering (POS) is a common complication following spinal anesthesia. Several drugs have been used to prevent POS, including ondansetron and pethidine. This study was conducted to compare the efficacy of ondansetron 8 mg with pethidine 0.4 mg/kg BW for prevention of shivering in pregnant patients undergoing a cesarean section with spinal anesthesia. This is a double blind controlled trial involving 96 pregnant patients between the age 18-40 years with ASA physical status I-II, gestational age of 37-42 weeks, body weight of 40-70kg or Body Mass Index (BMI) <30, body height >145 cm who underwent a cesarean section with spinal anesthesia in Dr. Sardjito General Hospital, Yogyakarta and affiliated hospital. Exclusion criteria included patients or families who refused to participate in the study, having a history of allergy to bupivacaine, ondansetron, and pethidin, patients with a fetus known to have congenital abnormalities earlier, body temperature early > 38 ºC or <36 ºC, pregnancy with complications (PEB, eclampsia, HELLP syndrome), and pregnant patients with heart disease (severe hypertension, heart trouble, abnormal heart valves). The patients were randomized into two groups i.e. 48 patients of group ondansentron receiving intravenous ondansetron 8 mg and 48 patients of group pethidine receiving intravenous pethidine 0.4 mg/kg BW. The patients were observed for occurence and severity of POS, postoperative nausea and vomiting. The results showed that two patients (4.2%) on the ondansetron group and six patients (12.5%) on the pethidine group experienced of POS. Moreover, the efficacy of ondansetron in the prevention of POS (95.8%) was higher than pethidine (87.5%). However, there were not significantly different (p>0.05). The incidence of nausea on the ondansetron group (4.2%) was lower than the pethidine group (16.7%) (p<0.05). However, no significant difference in the incidences of vomiting was observed between the ondansetron (0%) dan the pethidine (4.2%) groups (p>0.05). In conclusion, the efficacy of ondansetron 8 mg is comparable to pethidine 0.4 mg/kgBW for prevention of shivering in pregnant patients after spinal anesthesia. Background: Hypotension is one of the major adverse effects seen in patients undergoing spinal anaesthesia and is reported to be seen in more than 30 percent of the cases of patients receiving spinal anaesthesia. It has been highlighted in various studies that the use of atropine and ephedrine in the improvement of hemodynamic parameter when used preoperatively in spinal aesthesia. Hence; we carried this study to evaluate and compare the heart rate and mean arterial pressure after spinal anaesthesia with prophylactic use of atropine, ephedrine and placebo in elderly patients. Materials & Methods:The present study was conducted among 120 patients who underwent surgical procedures under spinal anaesthesia. Patients were randomly divided into three study groups; Group 1 (40 patients): who received normal saline, Group 2 (40 patients) who received 0.6 mg of atropine and Group 3 (40 patients) who received 12 mg of ephedrine. Study drugs were administered to the patients one minute after the spinal anaesthesia. Recording of the various parameters such as amount of vasopressor needed (phenlyephrine), after 15 minutes of spinal anaesthesia, sensory level reached presence of intra-operative angina and intra/postoperative confusion and other side effects till six hours post-surgically was done. The aim of this meta-analysis was to evaluate the preventive efficacy and safety of 5-HT receptor antagonists (5-HT RAs) on perioperative shivering. Relevant databases were searched to identify eligible randomized, controlled trials through January 2016. Primary outcome was the incidence of perioperative shivering, and secondary outcomes were the incidence of safety-related outcomes including postoperative nausea and vomiting (PONV), bradycardia, and hypotension. We calculated risk ratios (RRs) with 95% confidence intervals (CIs) for dichotomous data. Trial sequential analysis was performed to assess the risk of random errors and calculate the required information size. Sixteen studies with a total of 1126 patients were included in the meta-analysis. Compared with the control group, 5-HT RAs administered intravenously could statistically significantly reduce the incidence of perioperative shivering (RR, 0.44; 95%CI, 0.35 to 0.56; P < .00001; heterogeneity: I = 30%) as well as PONV (RR, 0.52; 95%CI, 0.28 to 0.97; P = .04; heterogeneity: I = 0%). However, they did not show superiority in lowering the rate of bradycardia (RR, 0.75; 95%CI, 0.38 to 1.49; P = 0.42; heterogeneity: I = 0%) or hypotension (RR, 0.79; 95%CI, 0.44 to 1.43; P = .44; heterogeneity: I = 24%). Trial sequential analysis of primary outcome showed that the required information size was 2634 patients and that the trial sequential monitoring boundary was crossed. Thus, more high-quality randomized, controlled trials with larger sample sizes are still required to draw a definite conclusion about the preventive efficacy of 5-HT RAs on perioperative shivering prevention in the future.
