Comparison of Open Drainage Versus Closed Catheter Irrigation for Treatment of Suppurative Flexor Tenosynovitis

Abstract: Surgical treatment of SFT with either OD or CCI resulted in similar outcomes for pain, function, and need for reoperation. Factors that lead to poor outcomes include S aureus culture, prolonged time to antibiotics, and smoking.

“…4,5 Similarly, although we found no associations between smoking status or swab culture results and reoperation rates, both parameters have be shown to negatively affect longer term outcomes. 3 A limitation of this study is potential confounding from unreported variables including latent period from injury to surgery along with medical comorbidities, both of which have been shown to affect functional outcomes. 4,5 Nevertheless, given that our primary outcome of interest was the requirement for further washout, which is more likely to reflect the adequacy of primary operation, we feel the conclusions remain valid.…”

“…Causative agents included TB 103 , NTM 109 , fungi 115 , and bacteria 118,119 . Pyogenic tenosynovitis was observed to have worse outcomes when the organism was Staphylococcus and when antimicrobial therapy was delayed 119 . Kanavel signs were observed to be sensitive but not specific 120 , and in 1 center's experience, when rice bodies were found in chronic tenosynovitis, the causative organism was often NTM, fungus, or unidentified 113 .…”
