Comparison of Outcomes for Children With Cervical Spine Injury Based on Destination Hospital From Scene of Injury

Abstract: Background: Pediatric cervical spine injury is rare. As a result, evidence-based guidance for prehospital triage of children with suspected cervical spine injuries is limited. The effects of transport time and secondary transfer for specialty care have not previously been examined in the subset of children with cervical spine injuries.

“…Currently, evidence supporting benefits of direct admission is limited, 44 as most SCI studies examined the benefits of early admission instead (i.e., within 24 h post-injury), and none stratified the injury by distance to SCI center. 2 , 4 , 5 , 10 However, direct admission for injuries within 40 km of a SCI center, and hence early access to care, offers many opportunities that would position patients and the healthcare system to gain substantially in the long term.…”

“…When establishing an ‘appropriate’ destination facility, paramedics must determine the likelihood of the patient requiring specialist services (personnel and equipment) and whether these will be available by the time the ambulance arrives. Almost 50% of patients initially transported to a more distant facility were within 15 km of the nearest hospital (43%), but it was twice as likely to occur outside of normal business hours (Monday to Friday, 8 a.m. to 6 p.m.).…”
