Objectives:The purpose of this study was to compare the retention of different three retention mechanisms (metal housing with nylon cap, PEEK and retention sil) in ball attachment retained mini-implant mandibular over denture.Methodology: A total of fifteen completely edentulous male patients were selected from outpatient clinic of the prosthodontics department. the patients were then randomly divided into three groups as five patients per group. Group I: overdentures retained by conventional metal housing and nylon caps, Group II: overdentures retained by silicone housing retention sil, Group III: overdentures retained by PEEK housing. All patients were scheduled for retention evaluation in a regular follow up visits at time of insertion, 3 months and 6months. Results:The results revealed there was a statistical significance increase in retention values in Group III : overdentures retained with PEEK housing when compared to Group I and Group II and the least retention values were recorded in Group II: overdentures retained with silicone housing retention sil at throughout the whole follow up period, While the comparison between different intervals revealed insignificant difference in all groups as P>0.05 (retention insignificantly decreased in all groups) at all intervals. Conclusion:Retention sil silicon housing is no longer reliable technique for retaining implant mandibular overdentures. While the PEEK housing provided the highest retention values when compare with the metal housing and silicon housing in ball attachment retained mini-implant mandibular over denture.

Purpose: To compare masticatory efficiency (ME) as an indicator for difference between barassisted overdenture versus fixed detachable denture wearers and an indicator for the effect of gender and period of edentulism. Materials and Methods:Twenty patients, 11 males and 9 females, were divided into two groups. Group I received fixed detachable overdenture assisted by four implants mounted by the all-on-four concept. Group II received mandibular implant overdenture assisted by extended ballbar overdenture. ME was measured after 3 months of denture insertion (T1), six months (T2), and one year (T3).Result: there was a significant increase of ME between both prosthetic design groups at T1 (p=0.004). In group I, there was a significant increase in ME through all time intervals (p<0.01 and p=0.027 for T1-T2, T1-T3, and T2-T3 respectively. For group II, there was a significant increase in ME between T1-T2 (p=0.031). At T3, there was a significant increase in ME for patients kept edentulous for less than a year before restoration and two-year edentulous patients before restoration ( p< 0.01 and p = 0.02, respectively). Also, there was a significant increase in ME for two-year edentulous patients compared to patients who stayed more than two years edentulous before restoration (p=0.02). Males revealed an increase of ME at T3 compared to female groups (p< 0.001). Conclusion:Within the limitation of the study, ME revealed a better prognosis with fixed detachable denture wearers. Male has better ME prognosis. ME revealed better prognosis with patients have shorter period of edentulism before denture insertion.

The difference between the maxillary and mandibular arches has been reviewed, and the literature suggests that the mandibular arch is more challenging in which to achieve rehabilitation goals due to the arch mobility, tongue motion, limited keratinized gingival cover, and the decreased surface area when compared to the maxillary arch and palate [ 5 ]. It has been proven in a lot of studies that fixed prosthetics are preferable to patients when compared to conventional complete dentures and implant-retained prostheses, which coincides with our cases, because all of them refused to opt for a removable prosthesis despite the lower cost, and easier hygiene; therefore, they all elected to opt for implant-based rehabilitation.…”

A 5-Year Satisfaction Outcome Study of Patients Receiving Six-Implant-Supported Fixed Prosthesis

The aim of the study was to analyze the satisfaction of patients treated with a protocol of six-implant-supported fixed prosthesis (6IFP) throughout 5 years of service. This retrospective study collected the data of all patients who had full-arch rehabilitations using 6IFP and followed them for 5 years. After applying the research inclusion/exclusion strategy, 37 cases were finally included in the study. All the patients had no previous complete dentures because they were partially edentulous, not interested in pursuing complete denture rehabilitation, had immediate dental extractions, implantation used the 2-stage protocol, and there was minor peri-implant socket grafting. Cases with severe bone loss that required extensive grafting were excluded. A total number of 222 implants were placed in the maxillary or mandibular arches in a total of 37 patients. The data presented the satisfaction outcomes concerning mastication, phonetics, and comfort during the first 5 years of the recall plan. The former was achieved based on the clinical record reviews, follow-up visits, and recall phone calls at the preoperative stage as well as annually thereafter. The mean satisfaction rate was 94.5%, with a mean record of 8.21 ± 1.7 out of 10, there was no gender predilection significance, and no age range variation significance was validated. Regarding the smoking status, the t-test score exhibited no significant effect on phonetics and mastication (p = 0.12, p = 0.16, respectively), whereas comfort was found to be significantly affected (p = 0.03). The comfort level was found to be slightly less at the immediate postoperative period among smokers when compared to non-smokers. In conclusion, partially edentulous patients who received the rehabilitation plan of arch dental extractions, six immediate implantations, and delayed prosthetic loading were found to be highly satisfied.
