Comparison of Robotic and Laparoscopic Hysterectomy for Benign Gynecologic Disease

Abstract: Objective-Utilization of robotically assisted hysterectomy for benign gynecologic conditions is increasing. Using the most recent, available nationwide data, we examined clinical outcomes, safety, and cost of robotic compared to laparoscopic hysterectomy.Methods-Women undergoing robotic or laparoscopic hysterectomy for benign disease were identified from the United States 2009 and 2010 Nationwide Inpatient Sample. Propensity scores derived from a logistic regression model were used to assemble matched cohorts … Show more

“…However, in the Premier database, a given physician cannot be linked across hospitals; as such, physician volume is likely under-estimated and unreliable and, therefore, was omitted from the predictors of conversion analysis. In addition, while ICD-9-CM diagnosis code V64.41 ("laparoscopic surgical procedures converted to open procedure") has been used consistently in other database studies 21,[38][39][40][41] , the code may be underreported because it is not directly linked to reimbursement. Nonetheless, any under-reporting should be comparable between the RA and LS groups.…”

“…Gynecologists have increasingly adopted minimally invasive surgical approaches since the first laparoscopic hysterectomy in 1988 by Harry Reich in Pennsylvania [5]. Nearly 50% of all hysterectomies performed for benign indications are now accomplished through laparoscopy [6]. A laparoscopic approach generally offers less intraoperative blood loss, shorter hospitalization, fewer wound complications and faster return to baseline activity for both hysterectomy and myomectomy compared with laparotomy [79].…”
